Can we help?
A Guide to Our Services
At Holmleigh we work closely with Service Users, family members, key support professionals and stakeholders, to determine which services would not only best meet an individual’s support requirements and interests in life – but would help them to achieve the greatest possible progression.
As well as helping our Service Users to improve their confidence and life skills, our support staff can also help Service Users to obtain and attend college placements, search for employment opportunities or to take part in their favourite leisure activities which may include anything from swimming, walking and football, to craft work and socialising.
All our support services are person centred, outcomes-focused and promote greater independence through life skills development. We have even developed the specialist staff teams and accommodation required to help those with more complex and challenging behaviour achieve positive outcomes in their lives – Please click on our individual services:-
This approach ensures that both the environment and care teams are well equipped to meet an individual’s specific needs- ensuring positive outcomes are met. We have many examples of developing our Service User’s life skills to such a degree that they have been able to choose to move onto a Supported Living home with reduced levels of support and greater independence
At Holmleigh Care, we can provide person centered support – focusing on achieving positive outcomes in all areas of daily living.
Our well-trained support staff help people to achieve greater independence through increased life skills development and can assist with everything from completing tenancy agreements to attending college, training opportunities and seeking employment.
In many cases Supported Living refers to shared accommodation but when a Service User is ready there is the choice to move onto their own flat with their own individual tenancy. Once again their key workers, co key workers and day support services are able to move with them reducing the trauma and ensuring the move is successful.
Supported living services are provided through Care at Home (our domiciliary care agency).
When the time is right, we will support our Service Users to find and apply for their own accommodation with their own individual tenancies (often in their own flats). Once they have secured the accommodation we will continue to support them with all aspects of their daily living requirements – continuing to achieve the positive outcomes specified in their support plans.
We help people who wish to remain living in their own homes by providing them with individual and tailored support when they need it most. All support packages are person-centred and can be flexible, results-focused and range from a 30 minute call to 24 hour care.
Over the years Holmleigh Care has proven time and again that it can support adults with complex and challenging behaviours. We specialise in supporting adults in the autistic spectrum with challenging behaviours in both residential and supported living settings.
There are over a million people in the UK who care for a loved one on a full-time basis. This can be a physically and mentally demanding role, even for someone who has been doing it for many years. We are often able to provide respite care, allowing family members or carers to have the occasional break. This can last anything from a few days to several weeks and allows carers to return to their duties feeling rejuvenated and refreshed.
If required, Holmleigh can offer to find appropriate accommodation outside its own organisation. Our staff will work closely with third party landlords and housing associations to find a vacancy which is the best fit for a Service User’s individual needs and circumstances. Holmleigh complies with the Reach Standards.
All our Service Users are welcome to get involved in visiting / helping out at a working farm. This has proved to be a real benefit and therapeutic for those who enjoy large outdoor spaces working with animals – in a safe environment.
In order to underpin our Service Users’ progression we offer a continuing service for life and ensure that all our services promote:-
- Fully trained staff teams
- A safe and secure environment
- Person centred planning
- Co-designing of support plans
- Life skills development
- Personal empowerment
- Greater wellbeing
- Greater independence
- Outcome focused planning/positive results
We also have our own accredited Training department, including three fully qualified PBM (Positive Behaviour Management) trainers, and our own Quality assurance managers. Together they make sure that all our staff teams are fully trained and supported, that progression is made and that services continue to maintain and improve on already high standards. We are also regularly audited by Social Services and the Care Quality Commission (CQC).
