  • When a prospective resident is referred, an assessment will be carried out which will look at all of the person’s needs.
  • Those needs will be looked at in the light of what the home can provide, taking account of the people currently living there.
  • On admission there will be a three month trial period during which time a care plan and day activity plan will be drawn up.
  • The home will develop Individual Person Centred Plans for each person with full involvement from the individual.
  • Prior to admission there will be liaison with all the professionals involved with an individual.
  • Care plans and behaviour management will reflect a working partnership between the individual, the home, parents/carers/advocates and involved professionals (e.g. GP, consultant, psychiatrist).
  • Each person will be encouraged and supported to participate in any local community activities in which they show an interest.
