Person Centred

Experience tells us that we achieve the best possible outcomes and progression for our Service Users when they have the greatest say in how they are supported. This is why we put person centred support planning at the heart of everything we do.

We know that by working closely with our Service Users and their key support professionals / stakeholders, together we can co-design meaningful support plans. These will underpin Service Users’ interests, progression and personal development in life by clearly specifying the outcomes they wish to achieve and the daily tasks that are necessary in order to ensure these outcomes are met.

A co-designed support plan delivered in a format that is understood and agreed by the Service Users, will help them to feel empowered and to engage in progressive support. It will also help them to pursue their interests in life and form the basis of how we should best provide the most effective support for that individual person.
